In order to provide and coordinate health care services to a defined population, a dynamic and systematic collaborative approach had to be adopted. Thus, the nursing case management system was developed, its framework composed of the following five essential elements:

Why Case Management Certification is Important

As a nurse case manager, you need case management certification since many employers prefer those with certification documents to show they are qualified for the job.

In addition to increasing employment opportunities, case management certification also serves as your best proof that you are qualified to apply the techniques and methodologies in nurse case management.

What a Nurse Case Manager Does

The work of a nurse case manager necessarily entails active participation with the clients with the aim of identifying and facilitating options and services for meeting individual health needs.

This would in turn aid in decreasing fragmentation and duplication of care. As such, the quality and cost effectiveness of clinical outcomes are enhanced. By doing all this, the nurse case manager brings a rich tradition of providing and coordinating comprehensive care not only to the client, but also to his family and the community in general.

Eligibility Requirements for Nurse Case Management Certification

Here are the general requirements that you must comply with in order to become eligible for initial case management certification:

* Meet all of the requirements for basic eligibility and any practice-specific eligibility requirements. If you fail to meet any of the requirements, you will be deemed ineligible to take the case management certification examination.
* Hold a diploma, associate, baccalaureate, or higher nursing degree. Proof in the form of either a copy or your diploma or transcripts with the degree conferred must be submitted. In addition, this information must be indicated in the application packet.
* Hold a current, active, unrestricted professional RN license in the United States or its territories.
* Have practiced the equivalent of two years full time as a registered nurse in the United States or its territories
* Have a minimum of 2,000 hours of clinical practice in the specialty area within the last three years.
* For practice occurring while in the military, in the Department of Veterans’ Affairs System, or outside the United States, an active RN license in the United States or its territories is required.
* Have completed 30 contact hours within the last three years
* Verification request (optional)
