Applying for Disability Benefits
If you are applying for disability benefits to social security, you have to do it the soonest possible time after becoming disabled. You have three ways in applying for disability benefits. You can apply online, calling on SSA toll-free number, or calling or visiting your local Social Security office.
In applying for disability benefits for an adult, you are required to complete an application for Social Security Benefits and an Adult Disability Report forms. The report collects all the details about the disabling condition and how it affects the ability to work. You can complete the forms online or you can call the Social Security office to schedule an appointment so that they can help you personally, or by phone.
Online Application for Disability Benefits
If you are applying for disability benefits online, go to ssa.gov and follow this procedure:
§ First, you have to review the Adult Disability Starter Kit. The kit answers common questions regarding applying for benefits and it also includes a worksheet that will help you gather the information you need.
§ Fill out the online application for Social Security Benefits. But, if you have never worked, skip this step and contact Social Security after completing step 3.
§ Fill out the online Adult Disability Report. At the end of the report, you will have to sign a form that gives your doctor the permission to send information about your disability to Social Security. The information is relevant in making up a decision on your disability claim.

Claims for disability benefits will take more time, which is about 3 to 5 months, than other types of social security claims. Although, the length of time to process your claims can be shortened if you have certain information and documents available with you when applying for disability benefits, and by helping out to get any other medical evidence you need to prove you are disabled. For example, you are to sign papers that would permit your doctor send your medical record to social security proving your disability.

§ Your Social Security number and proof of your age
§ Names, addresses, and phone numbers of doctors, caseworkers, hospitals, and clinics that took care of you and the dates of your visits.
§ Names and dosages of all the medications you are taking
§ Medical records from your doctor, therapists, hospitals, clinics, and clinics that you already have in your possession
§ Laboratory and test results
§ A summary of where you worked and the kind of work you did
§ Your most recent W-2 form, or if you were self-employed, a copy of your federal tax return.
For Family Members
§ Social Security numbers and proof of age for each person applying for disability benefits
§ If you are applying for disability benefits for your spouse, you should have proof of marriage, including the dates of prior marriages if applicable.
